Duodenal atresia

Sx: Newborn with bilious vomiting, down syndrome, and maternal polyhydramnios, classic double-bubble sign – gaseous distention of the stomach and enlarged duodenal bulb or mega bulb. No intestinal gut is seen distal to the duodenum.

Without distal bowel gas = duodenal atresia??

Duodenal atresia, also known as duodenojejunalatresia, is the congenital absence or complete closure of a portion of the lumen of the duodenum. It causes increased levels of amniotic fluid during pregnancy (polyhydramnios) and intestinal obstruction in newborn babies.

Explanation: The proximal left-sided bubble is the air- and fluid-filled stomach. The proximal duodenum represents the second bubble to the right of the midline.
